If you want to take advantage of these benefits, simply complete the pre-filled standard choice form and return it to your payroll department.

Otherwise, you can download a blank copy of the ATO superannuation standard choice nomination form to complete and submit to your employer.

Complete and return your Equip Super member options form so you can provide us with all your details, update your beneficiaries and make changes to your investment options and insurance.

Alternatively, once your member account has been created you can easily do this online by logging into your account (following the first contribution from your employer).
