Making a claim

Let us know how we can help

If you or your dependants need to lodge a claim the first step is to contact our Helpline on 1800 682 626.

We can then verify that your cover was valid and send you a package containing the forms that need to be completed. This should then be returned to the following address: Equipsuper, GPO Box 4303, Melbourne Vic 3001.

Depending on the nature and complexity of your claim, you may be asked to provide additional forms from your doctor, Centrelink, your employer and other third parties. The quicker you can supply the required information the quicker we can lodge your claim.

Remember, if you want to make a claim, contact us first. The vast majority of claims are settled without fuss and without the need for legal intervention.
