The forms available on this page are sorted into categories. Some forms are common to all members and are listed in the top panel. Forms specific to particular categories of member are listed separately under their respective headings.

If you are joining a new employer and want to advise them to pay your future contributions to Equipsuper, you will need the forms under Choice of Fund at the bottom of the page.

We do not provide Membership Application, Membership Options or Insurance Application forms for download from this site. This is because it is a legislative requirement that members joining the Fund must receive a relevant Product Disclosure Statement (PDS) and a Financial Services Guide (FSG) with these forms. If you are considering joining Equipsuper Personal as a superannuation or allocated pension member, you can access the forms by downloading the PDS from this website. If you are an Equipsuper Corporate member, you should have received an information pack including the form appropriate to your membership when you commenced work with your new employer. If you did not receive a Membership Application or Membership Options form from your employer, you can contact our Helpline 1800 682 626 to obtain one.
